Due to our continuing growth, we are now actively recruiting a Community Support Specialist to join our fine family of professionals.
The Community Support Specialist is responsible to provide our guests with meaningful and productive days in their own environment. The Specialist supports our Guests as they go about their daily lives that consist of a mix of employment, leisure/recreation, volunteer opportunities, community participation, and educational pursuits. Our highly-trained associates work closely with each guest and their family/care provider to determine best-fit growth opportunities and implement a customized life plan that provides invigoration that fosters success. The ideal Community Support Specialist has at least two years of human services support experience, post high school education that includes social services or human services coursework, a genuine desire to work in the developmental disabilities arena, and a willingness to work a flexible schedule including evenings, weekends, and/or holidays if needed. The following is a representative list of the duties and responsibilities associated with this position:
-- Support Guests according to their Individual Service Plan -- Provide training on activities of daily living skills/community integration activities to individuals with various cognitive levels and symptomology using an individualized approach -- Provide support to individuals in the maintenance of their homes including such tasks as: cooking meals, shopping, cleaning and laundry -- Assist individuals in self care, hygiene and activities of daily living; including assisting individuals in the bathroom, showering and personal self care -- Track and document an interface with local pharmacies to ensure proper medication supplies and storage for all program participants -- Provide monthly progress reports to ensure consistency between goals reported and objectives in the service plan and any treatment plans -- Facilitate and provide input during formal treatment plan meetings -- Provide effective service delivery and communication to people of varied cultural backgrounds who may not speak English as a first language -- Lead and/or co-facilitate groups and/or training to residents and families in support of program objectives -- Use Sharepoint Log system to document all communication and shift notes as required by program procedures -- Maintain maximum safety conditions and cleanliness standards at all times within the appropriate areas in accordance with licensing standards and program procedures. Ensure outside areas are safe and free of snow and ice during winter months to warrant safe evacuation from the house at all times. Must be able to operate a motorized snow blower -- Adhere to Residential Options policy and procedures with regard to development and maintenance of individual charts and clinical/behavioral and health -- Accurately and promptly record financial information.
