Fort Norfolk Retirement Community, Inc./d.b.a Harbor's Edge is a non-profit Continuing Care Retirement Community ("CCRC") located in historic Fort Norfolk, the site of one of 19 harbor-front forts authorized by President George Washington in 1784.  Harbor's Edge is a 5 star CCRC with 163 independent living apartments, 33 standard assisting living apartments, 17 demential assisting living apartments and 33 skilled nursing beds constructed on approximately 4.3 acres located on the waterfront to provide a quality senior living option for Norfolk Seniors.

JOB DESCRIPTION
A Licensed Practical Nurse (LPN) is a primary provider of resident care.  An LPN promotes the physical, personal and emotional well being of each resident and strives to maintain the residents at their maximum level of self-care and independence.  The ultimate goal of quality resident care is to enable each resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression.  Resident care is not passive maintenance; an active approach will always be taken.
The following are the general duties assigned to the LPN in Assisted Living Memory Support.  It is NOT all inclusive.

Provide direct nursing care services to residents.  Follow individualized service plan of care for all residents (ADL  Assignment Worksheets).
Perform resident assessments as required by the VDSS standards/ regulations and facility policy.
Observe residents for change in condition and report any changes immediately to nursing supervisor as well as resident’s physician as directed.
Document all pertinent/relevant observations, communication with the resident’s physician & responsible party, clinical procedures and daily assistance/care by the end of each shift.
Complete and maintain current individualized service plan of care for each resident.
Attend all scheduled interdisciplinary resident care meetings.
Check and record vital signs, weights, and blood sugars as assigned.
Administer medications, injections, and other treatments as authorized by current licensure by the VA. State Board of Nursing.
Provide for or assist patients with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc.  Perform restorative nursing techniques including proper positioning, ADL training, and Passive Range of Motion.  Assist in providing reality orientation, redirection, and validation techniques to residents in need.
Encourage residents to participate in scheduled facility activities providing assistance in getting to activities as needed. Assist in the planning and implementation of resident activities as needed.
Assist residents with transfers and ambulation as directed and ordered by the physician, using proper body mechanics.
Respond to urgent calls as needed.
Maintain restraint free environment at all times.  Support fall prevention programs for all residents in need.
Help admit and discharge residents according to facility procedure.
Assist in post-mortem care as instructed.
Perform other duties as assigned.  Attend in-service classes and staff meetings.  Orient new employees.
 
 
REQUIREMENTS
  Are you CPR and First Aid Trained and possess a valid certificate/card ?
  Must be physically able to lift upto 25 pounds
  Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, balancing and maintaining body equilibrium to prevent falling, reaching over the head, pushing/pulling, grasping~both hands
  Must be able to work nights, weekends, holidays and during inclement weather events
  Current state license as a LPN in the Commonwealth of Virginia
  High school diploma/GED at a minimum
  Minimum of one year experience at this level of responsibility
  Excellent verbal and written skills
  An up-beat and engaging personality
  Genuine desire to work with the elderly and/or disabled
  Able to follow written and oral instructions
  Desire to work in the Continuing Care Retirement Community (CCRC)
  Be able to deal tactfully with personnel, residents, family members, visitors, and the general public
  A heartfelt compassion for this line of work
  Minimum of one year experience in Assisted Living/Memory Support facility
